How to Complete Your Total and Permanent Disability Loan Discharge Application

It's important that you and your doctor fill out the application completely. If anything is missing or incomplete, we will return the application to you.

Step Number Procedure
Step 1:

Download and print the application.
Step 2: Complete all of Sections 1 and 3 in dark ink.
Step 3: Sign and date the application. (We must have your original signature. Photocopies are not acceptable.)
Step 4: Ask your doctor to take the steps the below.

Step 1:

Complete section 4 in its entirety, including:

  • The states where you are licensed to practice
  • The appropriate license number for each state
  • Your signature and date
Step 2:

Provide a diagnosis

  • Indicate the diagnosis on the application.
  • To provide additional information detailing how this condition prevents the borrower from working in any capacity, please attach a letter on your letterhead.
Step 3:

Explain corrections

If you make any corrections on the application, please attach a letter explaining why the changes were necessary. Changes may include:

  • Typographical errors
  • Human or record errors
  • New information

This letter must also include the signature of the person who made the change.
